Welcome to the Community Portal for the Collaborative Care Program

The Collaborative Care Program is a community-centred approach to addressing primary healthcare challenges in rural NSW. These challenges include providing access to primary care services, the recruitment and retention of health practitioners, and the sustainability of these services. The program is currently evaluating models of care in five project locations across rural NSW. This website has information on each of these projects and the various tools and methods used by local representatives to address the needs in these communities.

 

Open Projects

A single-employer model has been developed for the towns of Tottenham, Trundle, Tullamore and Trangie, sharing doctors and practice nurses across the four towns. The Collaborative Care Program will support activities to improve the sustainability of the model.

Since 2018, Dr Ros Bullock, a local GP in Canowindra has been trialling a deliberate team-based care model (DTBC) for patients with a high risk of health decline. The Collaborative Care Program will evaluate the evidence for improved health outcomes.

The Collaborative Care Program in the Lachlan Valley (Parkes, Forbes and Condobolin) is focused on the development of the health workforce across the region and opportunities to coordinate and share services.

The project is trialling group medical appointment models for patients with chronic health concerns in general practice settings in the Snowy Valleys and the neighbouring Riverina area.
