Wee Waa

Health services are delivered by a lot of different organisations. Some are public organisations like hospitals, or private businesses like GP clinics and chemists, and some are charities. They all provide care to your community.

The Wee Waa Collaborative Care project aims to identify solutions to primary healthcare needs through a co-design process. Representatives from local health service providers and community organisations are working together to support the general health and well-being of the community in Wee Waa and its neighbouring towns.

We are inviting the community to have their say on local health service needs through a Community Health Services Survey.

  • Wee Waa Community Health Services Survey

    This confidential survey will ask about your experiences using health services over the past 12 months. The survey takes around 10 minutes to complete. Each member of your household who is 15 years of age and over can complete this survey. You can answer questions for yourself and others in your immediate family. If you are a carer, you can also speak on behalf of someone you care for. Many questions allow you to provide comments to add more information about your experiences.

    By understanding your experiences, your local health care and social care providers will be able to work together to better plan for the needs of your community.
