Health services are delivered by a lot of different organisations. Some are public organisations like hospitals, or private businesses like GP clinics and chemists, and some are not-for-profit organisations like an Aboriginal Medical Service. They all provide care to your community.

The Collaborative Care project in Liverpool Plains will support access to primary health care services for the community, including GP services. The project is being supported by Liverpool Plains Shire Council, local health providers, and community support agencies.

We are inviting the community to have their say on local health service needs through a Community Health Services Survey.

The Collaborative Care Program received funding through the NSW Ministry of Health.

FAQ

Primary health care is generally the first point of contact you have with the health system. This might be your local GP, or an allied health professional such as a dentist, physio or speech pathologist. Your local pharmacist is another primary health care provider, and so is a community nurse or a midwife. These health practitioners provide continuing care to patients and coordinate any specialist care the patient may need.

The Collaborative Care project is supported by representatives from HealthWISE, Hunter New England Health, Liverpool Plains Shire Council, Primary Health Network, Rural Doctors Network, Tamworth Aboriginal Medical Service, and WalHealth.
