The 4Ts

A single-employer model is being piloted across the towns of Tottenham, Trundle, Tullamore, and Trangie creating a network of general practitioner (GP) and primary care services. Western NSW Local Health District shares doctors, nurses, and administrative staff across the four towns, coordinated by a medical centre manager. The clinics are co-located within the town Multipurpose Service (MPS). The Collaborative Care Program is supporting activities to improve the sustainability of the model and to share these learnings with the rural health sector.

Regular collaboration with local communities is critical to the project. This includes staff, health councils, local government, and community representatives working with the Western NSW Local Health District (WNSWLHD), Western NSW Primary Health Network (PHN), and NSW Rural Doctors Network (RDN).

This innovative pilot project is being evaluated through the Collaborative Care Program. While the implementation is ongoing, the aim is to understand how well the shared GP service has addressed community needs and to refine the model to make it financially sustainable.
