Since 2018, Dr Ros Bullock, a local GP in Canowindra has been trialling a deliberate team-based care model (DTBC) for patients with a greater risk of health decline. These patients are case managed by a multidisciplinary team representing general practice, specialists, pharmacy, paramedicine, nursing, allied health, and aged care.

The Collaborative Care Program will evaluate any evidence for improved health outcomes and lessons learned from the trial. The Program will also consider the perspectives of other local GPs and alternate methods of primary health care for patients with chronic or complex conditions.

Rural Health is a Team Sport

Dr Bullock and her team help patients with chronic health conditions. The initiative is part of the Collaborative Care Program in Western NSW.
