Since 2018, Dr Ros Bullock, a local GP in Canowindra has been trialling a deliberate team-based care model (DTBC) for patients with a greater risk of health decline. These patients are case managed by a multidisciplinary team representing general practice, specialists, pharmacy, paramedicine, nursing, allied health, and aged care.

The Collaborative Care Program will evaluate any evidence for improved health outcomes and lessons learned from the trial. The Program will also consider the perspectives of other local GPs and alternate methods of primary health care for patients with chronic or complex conditions.

Dr Ros Bullock, a local GP in Canowindra, has been trialling a deliberate team-based care model for patients with a high risk of health decline. Dr Bullock and her team help patients with chronic health conditions. The initiative is part of the Collaborative Care Program in Western NSW.

Primary health care is generally the first point of contact you have with the health system. Typical examples are your local GP, or an allied health professional such as a dentist, physio or speech pathologist. Your local pharmacist is another primary health care provider, and so is a nurse or a midwife. These health practitioners provide continuing care to patients and coordinate any specialist care the patient may need.

The Deliberate Team-Based Care model in Canowindra is led by Dr Ros Bullock and involves a multidisciplinary team of local health professionals. The Collaborative Care Program partners who are jointly evaluating this project are Western NSW Local Health District, Western NSW Primary Health Network, and Rural Doctors Network.

The Program has received funding through the Innovative Models of Care grant program that is administered by the Commonwealth Government Department of Health and Aged Care.
