  • Rural Health is a Team Sport

    Dr Ros Bullock, a local GP in Canowindra, has been trialling a deliberate team-based care model for patients with a high risk of health decline. These patients are case managed by a multidisciplinary team representing general practice, specialists, pharmacy, paramedicine, nursing, allied health, and aged care.

    Dr Bullock and her team help patients with chronic health conditions. The initiative is part of the Collaborative Care Program in Western NSW.

    "We very deliberately wrap our care around a patient," said Dr Bullock.

    "That means the decisions that are made and the reason for getting together, the things that are discussed, are all to do with improving the care of that person."

    "There is a lot of time required to get the team together and to make it all happen but I think once you invest in that the rewards definitely outweight the drawbacks."

    The Collaborative Care Program is currently evaluating the model to determine whether this intentional collaboration between health practitioners has improved the health outcomes of patients.
