Snowy Valleys

The Snowy Valleys Project addresses local access to primary health care, which encompasses general practitioners, allied health professionals, primary care nurses, pharmacists, and Aboriginal and Torres Strait Islander health workers.

The project is focused on multidisciplinary care for chronic disease management and is currently trialling Group Medical Appointment (GMA) models in the Snowy Valleys and the neighbouring Riverina area.

FAQS

Primary health care is generally the first point of contact you have with the health system. Typical examples are your local GP, or an allied health professional such as a dentist, physio or speech pathologist. Your local pharmacist is another primary health care provider, and so is a community nurse or a midwife. These health practitioners provide continuing care to patients and coordinate any specialist care the patient may need.

The Snowy Valleys project is a partnership between Murrumbidgee Local Health District, Murrumbidgee Primary Health Network, and NSW Rural Doctors Network. We are currently supporting general practices in the Snowy Valleys and Riverina area to trial group medical appointments for patients with chronic health concerns.

The Program has received funding through the Innovative Models of Care grant program that is administered by the Commonwealth Government Department of Health and Aged Care. It is currently funded until June 2024 to trial a group medical appointment model in the Snowy Valleys and the Riverina area for patients with chronic health conditions.
