Lachlan Valley

The Collaborative Care Program in the Lachlan Valley (Parkes, Forbes and Condobolin) is focused on the development of the health workforce across the region and opportunities to coordinate services. The project brings stakeholders together from the three communities to identify primary health needs and work in a co-design process to come up with solutions that will improve access to primary health care. This includes the development of a Health Workforce Strategy for the region and a targeted GP recruitment campaign to support models of care.
